It’s Not Just Perimenopause
Perimenopause doesn’t just cause fatigue, weight gain, or stubborn weight loss resistance. Instead, it removes the buffers that allow other issues to come forward.
Estrogen keeps inflammation in check
Estrogen helps maintain immune tolerance, the system that keeps your immune cells from attacking your own tissue, or from developing food intolerances.
As estrogen becomes erratic in your forties, that tolerance gets shaky, and autoimmune conditions that were simmering quietly start making noise.
This is why so many women get a Hashimoto’s diagnosis in the same decade the hot flashes start.
It’s also why the two get confused. Fatigue, brain fog, weight gain, broken sleep, low mood, thinning hair — those are symptoms of both low thyroid and perimenopause.
Your blood sugar changed too
Estrogen improves insulin sensitivity. When it drops, you are more prone to blood sugar crashes, spikes, and insulin resistance.
Unstable blood sugar drives inflammation. Inflammation makes autoimmune activity worse.
Blood sugar is usually ground zero for a lot of what we see clinically. Fatigue, unexplained weight gain, stubborn weight loss resistance, and even hair loss can all have their roots in blood sugar issues.
Women often see their lipid panels also worsen with both perimenopause and low thyroid.
What to ask for
A TSH alone does not tell you whether your immune system is attacking your thyroid. Ask for:
- TPO and thyroglobulin antibodies
- Free T3 and free T4, not just TSH
- Fasting insulin and hemoglobin A1c
- hs-CRP
- Vitamin D and ferritin
Most of these are inexpensive, and most doctors will run them if you ask directly.

Supplements worth starting with
I get asked constantly what supplements to take. While labs and symptoms dictate a personalized protocol, four things are foundational (and found on my Palm Life shop):
